Before you can try out or participate:

  1. Athletic Clearance Confirmation Page printed & signed from homecampus.com (the confirmation page is the last item in the athletic clearance process, and serves as proof that you completed it).

    Girls: Please print select *both* Indoor *and* Beach volleyball, even if you only want to do one - some practices may overlap.

Important:

  • You will need to upload a copy of your medical insurance card to the homecampus.com athletic clearance website.
  • You will *not* need to upload a copy of your physical exam form to the homecampus.com athletic clearance website - the original needs to be turned in.
  • Please make of copy of all forms for your records, and place the orginals in Athletic Director Richard Martinez's box in the main office. Emailed subbmissions and photocopies of these forms cannot be accepted.
  • Late paperwork can cause delays in playing time. Get it in early - paperwork needs at least a week to be cleared by the school administration once it is turned in.

The school district provides a school bus for only 7 (out of usually 10) away Indoor matches a year. There are no buses provided for weekend tournaments, Beach contests or any other off-site events, including off-site practices. Carpooling is an absolute necessity.

**You will need to include a copy of your drivers license and proof of automobile insurance.

These transprtation froms can be scanned and emailed to the appropriate coach, or dropped off in thier box in the main office.

A couple of important carpool rules:

  • No adult - coach, staff, parent or volunteer - can ever be alone in a car with a child other than thier own.
  • A student with a valid drivers license may drive themself to an event, but may not drive any other students.
  • One other frequently needed form:
  1. Parent Pick-up Letter - this form allows a parent or guardian to pick up thier child from an away game in lieu of her returning to ERHS on the school bus. | Download Blank Form

This form must be signed off by an ERHS administrator at least 24 hours before the date of the game you wish to pick up your student from. No Exceptions. Please bring the form to school (or have the student bring it), walk it to Ms. Jennifer Tsui, the Assistant Principal in charge of athletics for her signature, and then place it in your coach's mail box in the main office. If Ms. Tsui is unavailable, a signature from another AP will do.
